Diagnosed with CIS suggestive of Multiple Sclerosis (MS) in December 2024; previously diagnosed with Functional Neurological Disorder (FND).

PSP initially diagnosed as FND
A patient developed progressively worsening dysphonia, gait instability, falls and ocular motor symptoms. An initial diagnosis of functional voice disorder (within the functional neurological disorder spectrum) was made after largely normal neurological assessment and MRI. Despite rehabilitation, symptoms continued to progress. Repeat neurological assessment, MRI susceptibility imaging and DaTscan demonstrated findings consistent with progressive supranuclear palsy (PSP), leading to revision of the original diagnosis.

GSS initially diagnosed as conversion disorder
A woman first presented to neurology in January 2019 with intermittent tremor, dysarthria and psychomotor slowing. Brain MRI, EEG and lumbar puncture were unremarkable, and conversion disorder was diagnosed. Over the following years she developed progressive speech impairment, gait disturbance, recurrent falls, cognitive deficits and objective abnormalities on imaging, EEG and neuropsychological testing. In January 2023, molecular genetic testing identified a heterozygous nine-octapeptide-repeat insertion in PRNP, also present in her father, confirming Gerstmann–Sträussler–Scheinker disease (GSS). https://pmc.ncbi.nlm.nih.gov/articles/PMC12670835/
Sporadic CJD initially diagnosed as functional neurological movement disorder
A 64-year-old woman presented after approximately two months of worsening right-sided tremulous and jerky movements, numbness, ataxia, headaches and joint pain. During the first week of hospitalization, distractibility, variability and entrainment were interpreted as supporting a functional neurological movement disorder. She rapidly deteriorated; repeat EEG and MRI became abnormal, and brain biopsy confirmed sporadic Creutzfeldt-Jakob disease. The paper gives relative intervals rather than calendar dates.
